Vitality Plan 2012

We are pleased to offer our bespoke practice membership scheme. It is our way of ensuring that we care for you to the best of our ability and allows you to pay for regular dental care the easy way. The regular care you receive as a VitalityPlan patient helps ensure that your treatment needs are identified in a timely manner and therefore treatment is as successful as we can make it.

VitalityPlan £19.50 per month

  • Adults 18 years of age and over (21 and over if in full time education)
  • Two routine check-ups per year including x-rays (£122 if purchased separately)
  • 60 minutes of hygiene per year (£144 if purchased separately)
  • 15% discount on all treatment fees (7.5% on implant-related services)
  • Free oral cancer screen
  • Free subscription to ReVitalise, our quarterly newsletter with exclusive offers for VitalityPlan members
  • Worldwide trauma insurance to cover accidental damage
  • VitalityPlan Basic is available at £8.50 per month for patients with complete dentures or those who are on a bespoke hygiene treatment plan

VitalityPlan Kids £4.50 per month

  • Children from 5 to 17 years of age (and young adults up to 20 in full-time education)
  • One routine check-up per year (£32 if purchased separately)
  • One oral health education session with our dental therapist per year (£32 if purchased separately)
  • 15% discount on all treatment fees
  • Worldwide trauma insurance to cover accidental damage

Worldwide Trauma and Emergency Callout Insurance

Emergency callout – cover can be provided by any dentist worldwide who agrees to treat you. You simply pay their charge and collect a receipted invoice. This will then be processed through the practice for reimbursement from the insurance and you will be refunded the callout fee, minus the excess, up to the policy limits. This cover is to provide immediate pain relief only; no additional or restorative treatment fees can be claimed. You should return to the practice during normal hours for any further treatment required.

Dental trauma – cover is provided should you be unfortunate enough to suffer a dental trauma, for example, as a result of a road traffic accident or an accident at home or at work. The insurers will settle the claim up to the policy limits and you will need to pay any relevant excess.

Hospitalisation – benefit is provided should you have to stay in hospital as a result of the accident.

Oral cancer – a payment will be made upon diagnosis of oral cancer.

Redundancy protection – plan premiums paid for up to one year.

A leaflet giving details of the insurance cover, limits, excesses and exclusions is available at the practice. Practice staff are permitted to provide you with information about the insurance that is included in your plan on behalf of Practice Plan Limited, the plan administrator. We will complete an agreement with you to start your membership. This will require your signature and bank account details for the direct debit mandate. Direct debits are collected from your account on the 8th of every month. Benefits will commence from the 1st of the month (effective month on the dentist/patient agreement). Please note that VitalityPlan membership is not a contract and there is no minimum term or tie-in period.
